Nonimmigrant Visa Applications
- DS-160: Online Nonimmigrant Visa Application (for all nonimmigrant categories except K applications)
NOTE: K visa applicants applying at U.S. Consulate General Montreal have different application form requirements. Review information on the U.S. Consulate General Montreal website to learn which forms you must complete. K visa applicants applying at all other locations must complete Form DS-156.

- DS-2019: Certificate of Eligibility for Exchange Visitor Status. This form cannot be downloaded here. Contact your exchange visitor program sponsor, who is responsible for entering the DS 2019 information into SEVIS, and providing the SEVIS generated DS-2019. Important Note: The current DS-2019 form generated with an expiration date of April 30, 2008 remains valid at this time.

Immigrant Visa Applications
- DS-117: Application to Determine Returning Resident Status
- DS-156K: Nonimmigrant Fiancé(e) Visa Application
- DS-230: Application for Immigrant Visa and Alien Registration
- DS-260: Online Immigrant Visa Application (NEW: Limited Use): Required for visa applicants to apply at selected U.S. Embassies and Consulates after National Visa Center notification. This web-based electronic form will eventually replace the current Form DS-230, Application for Immigrant Visa. The roll-out of the new DS-260 will be gradual. Therefore, not all immigrant visa applicants will be required to use the new form at the same time. Go to Online DS-260 guidance for more information.
- DS-261: Choice of Address and Agent (NEW: Limited Use): Required for applicants at selected U.S. Embassies and Consulates after National Visa Center notification. This web-based electronic form will eventually replace the current Form DS-3032 Choice of Address and Agent. The roll-out of the new DS-261 will be gradual and happen along with the Online Immigrant Visa Application. Therefore, not all immigrant visa applicants will be required to use the new form at the same time. Go to Online DS-261 guidance for more information.

Important Note: The NVC cannot accept Form I-134 for immigrant visa processing. The NVC only accepts the appropriate I-864 form(s) listed above.
If you are looking for all other forms that begin with "I" (such as I-130, I-539, etc), these forms come from the U.S. Citizenship and Immigration Services (USCIS) in the Department of Homeland Security. You may download them from the USCIS forms page.
